Nurse Case Manager

o As an RN Case Manager at Mitten Care, you will be a vital part of our patient-centered care team, responsible for:
o Managing patients with complex care requirements resulting from auto and work-related injuries
o Conducting assessments, planning, and facilitating high-quality care, and advocating for patients and families to address their comprehensive healthcare needs
o Evaluating clients' physical and mental wellness, preferences, and abilities to develop tailored care plans ensuring the highest quality of care and patient safety

• Education: Bachelor's Degree in Nursing
• Licensure: Registered Nurse License in the State of Michigan
• Experience: Three (3) years of experience as a Case Manager or three (3) years of direct patient care experience as an RN with demonstrated clinical expertise
• Skills: Strong communication, collaboration, problem-solving, time management, and organizational skills

Bonus Qualifications:
• CCM (Certified Case Manager) certification preferred
