A Licensed Practical Nurse (LPN) at UHI supports individuals with intellectual disabilities to live as they choose. This includes assisting with health and hygiene needs, food preparation, grocery shopping, household tasks, financial management, and required documentation. This position requires ethical conduct, patience, reliability, effective communication, compassion, adaptability, and the ability to work independently.

What You'll Do

 ESSENTIAL FUNCTIONS AND BASIC DUTIES:

  • Uphold UHI’s mission, promoting dignity, respect, and choice for individuals served.
  • Support a stable living environment, ensuring health and safety while honoring individual rights and preferences.
  • Complete all duties as a Mandatory Abuse Reporter.
  • Respond effectively to emergencies or crises, which may require physical intervention.
  • Administer medications accurately as prescribed.
  • Maintain required certifications, including CPR, First Aid, and OIS, renewing every two years.
  • Complete a minimum of 12 hours of ongoing education biennially.
  • Facilitate community engagement through planned activities and outings.
  • Assist with transportation, ensuring safe vehicle operation and proper use of wheelchair tie-downs.
  • Document incidents, grievances, and communications accurately and confidentially.
  • Maintain a safe and supportive home environment, providing role modeling in personal care, hygiene, and daily living skills.
  • Implement programs, treatments, and support as directed by the RN or Program Manager.
  • Use mobile applications for communication, documentation, and scheduling (cellphone app downloads may be required).
  • Other duties as assigned

 

SKILLS AND ABILITIES:

  • Strong organizational and time-management skills.
  • Ability to follow medical protocols, including administering prescriptions and conducting diagnostic testing.
  • Effective data recording and documentation practices.
  • Commitment to teamwork and supportive care in all environments.
  • Proficiency in maintaining confidentiality and compliance with UHI policies and procedures.

What You Bring

  • Active, unrestricted State of Oregon LPN license in good standing.
  • Valid Oregon driver’s license with an acceptable driving record.
  • Ability to regularly push, pull, and lift a minimum of 50 lbs.
  • Successful completion of a State/Federal criminal history check and pre-employment drug screening for federally illegal substances.
